What are the terms, exclusions and limitations?
This is NOT dental insurance.
Can’t be combined with any dental insurance/discount.
Consumable dental products such as toothpaste, bleaching, and electric tooth brushes are not included in the plan.
Should there be dental treatment needed followed any type of injury where a lawsuit and therefore outside medical, car, disability, or workman’s comp type insurance are involved, this oral health/discount plan can’t be used.
Payment in full is due at time of service.
Membership expires exactly 12 months to the day of joining the the program and therefor member will be solely responisble for maximizing the plan benefits within the membership period.
Plan is subject to change yearly.
Must remain plan participant entire duration of recommended treatment.
Plan is non-refundable and non-transferable.
Membership fees are due the day of joining the plan and membership will become effective immediately upon receipt of dues.
